If you require a last minute refill for last minute travel, you missed doing your labs on time, or you lost your prescription - We will authorize 1 month script at a FEE of $75 - this is only for current patients with us. You must do your blood work & consult at your scheduled time & pay your regular follow-up consult fee.
Replacement Prescriptions or Lab Requisitions

At Integrative Health and Preventive Medicine we fax or email prescriptions and lab requisitions to the pharmacy and your preferred lab. This is included in the cost of the consultation and follow up appointments and helps avoid lost or misplaced needed paperwork. If you lose or misplace a prescription or lab requisition an additional fee will apply.
